Any of the three will find your account, and case does not matter here. Signing in is stricter — it takes your username or email exactly as they were registered — so use whichever you actually remember.
The 6-digit code goes to the email address on the account and is valid for 15 minutes. Requesting another one cancels the previous code, so if there are several mails in your inbox, only the newest one works. For security we give the same answer whether or not an account matched, so this is not a confirmation that a mail was sent. If nothing arrives within a few minutes, check the address on the account with an administrator.
